Third Party Authority Form


This form enables us to contact your existing fund(s) regarding your rollover to your new Superfund.

Authorisation by member
I hereby authorise Jonnah Varquez, Shelly Earnshaw, Justine Brown, Jasmine Stanley and James Ridley (authorised under AFSL #479263) to be the administrators of my superfund:

  1. To access and/or obtain copies of all information or documentation relating to my superannuation fund, my current investments and my insurance cover.
  2. To organize for duplicate or electronic copies of any investment statements, dividend notices, broker accounts or bank statements to be forwarded to them.
